Shortages of new therapeutics to treat coronavirus disease (COVID-19) have forced clinicians, public health officials, and health systems to grapple with difficult questions about how to fairly allocate potentially life-saving treatments when there are not enough for all patients in need (1). Shortages have occurred with remdesivir, tocilizumab, monoclonal antibodies, and the oral antiviral Paxlovid (2) -/- Ensuring equitable allocation is especially important in light of the disproportionate burden experienced during the COVID-19 pandemic by disadvantaged groups, including Black, Hispanic/Latino and (...) Indigenous communities, individuals with certain disabilities, and low-income persons. However, many health systems have resorted to first-come, first-served approaches to allocation, which tend to disadvantage individuals with barriers in access to care (3). There is mounting evidence of racial, ethnic, and socioeconomic disparities in access to medications for COVID-19 (4, 5). -/- One potential method to promote equitable allocation is to use a weighted lottery, which is an allocation strategy that gives all eligible patients a chance to receive the scarce treatment while also allowing the assignment of higher or lower chances according to other ethical considerations (6). We sought to assess the feasibility of implementing a weighted lottery to allocate scarce COVID-19 medications in a large U.S. health system and to determine whether the weighted lottery promotes equitable allocation. As clinical ethics consultants move toward professionalization, the process of certifying individual consultants or accrediting programs will be discussed and debated. With certification, some entity must be established or ordained to oversee the standards and procedures. If the process evolves like other professions, it seems plausible that it will eventually include a written examination to evaluate the core knowledge competencies that individual practitioners should possess to meet peer practice standards. The American Society for Bioethics and Humanities has published core knowledge (...) competencies for many years that are accepted by experts as the prevailing standard. Probably any written examination will be based upon the ASBH core knowledge competencies. However, much remains to be done before any examination may be offered. In particular, it seems likely that a recognized examining board must create and validate examination questions and structure the examination so as to establish meaningful, defensible parameters after dealing with such challenging questions as: Should the certifying examination be multiple choice or short-answer essay? How should the test be graded? What should the pass rate be? How may the examination be best administered? To advance the field of health care ethics consultation, thought leaders should start to focus on the written examination possibilities, to date unaddressed carefully in the literature. Examination models—both objective and written—must be explored as a viable strategy about how the field of health care ethics consultations can grow toward professionalization. (shrink)

This paper reframes the futility debate, moving away from the question “Who decides when to end what is considered to be a medically inappropriate or futile treatment?” and toward the question “How can society make policy that will best account for the multitude of values and conflicts involved in such decision-making?” It offers a pragmatist moral epistemology that provides us with a clear justification of why it is important to take best standards, norms, and physician judgment seriously and a clear (...) justification of why ample opportunity must be made for patients, families, and society to challenge those standards and norms. During public health crises including the COVID-19 pandemic, resource scarcity and contagion risks may require health systems to shift—to some degree—from a usual clinical ethic, focused on the well-being of individual patients, to a public health ethic, focused on population health. Many triage policies exist that fall under the legal protections afforded by “crisis standards of care,” but they have key differences. We critically appraise one of the most fundamental differences among policies, namely the use of criteria to categorically exclude (...) certain patients from eligibility for otherwise standard medical services. We examine these categorical exclusion criteria from ethical, legal, disability, and implementation perspectives. Focusing our analysis on the most common type of exclusion criteria, which are disease-specific, we conclude that optimal policies for critical care resource allocation and the use of cardiopulmonary resuscitation (CPR) should not use categorical exclusions. We argue that the avoidance of categorical exclusions is often practically feasible, consistent with public health norms, and mitigates discrimination against persons with disabilities. (shrink)

The coronavirus disease 2019 (COVID-19) pandemic has motivated medical ethicists and several task forces to revisit or issue new guidelines on allocating scarce medical resources. Such guidelines are relevant for the allocation of scarce therapeutics and vaccines and for allocation of ICU beds, ventilators, and other life-sustaining treatments or potentially scarce interventions. Principles underlying these guidelines, like saving the most lives, mitigating disparities, reciprocity to those who assume additional risk (eg, essential workers and clinical trial participants), and equal access may (...) compete with one another. We propose the use of a “categorized priority system” (also known as a “reserve system”) as an improvement over existing allocation methods, particularly because it may be able to achieve disparity mitigation better than other methods. Focusing on David B. Riazanov career and his pioneering efforts in producing a complete edition of the works of Marx and Engels, the article explains why Riazanov’s variety of Marxism was unacceptable to the Soviet regime, and why from 1924 Lenin was credited with being an outstanding Marxist theoretician, whereas previously he had been regarded only as a skilled political activist. The concept of Leninism as a new stage of Marxism was put forward by Bukharin and elaborated on by Stalin (...) and Zinoviev. Georg Lukács attempted to show that Lenin’s thought had the internal coherence that Bukharin postulated. In support of Lenin’s theoretical credentials, his ‘Philosophical Notebooks’ began to be published from 1925. However, neither Riazanov nor his associate Abram M. Deborin subscribed to the official doctrine that Leninism was a new and higher stage of Marxism. For this stance they fell victim to Stalin’s repression. (shrink)

Introduction: Early recognition of prediabetes may prevent progression to diabetes, yet not all adults are aware of their prediabetes risk. To reach all adults unaware of their risk, additional risk assessment strategies are warranted. Objectives: The objective of this study was to evaluate the potential scope of benefit from prediabetes risk assessment in the dental care setting and to identify characteristics of dental patients likely to unknowingly have prediabetes or diabetes. Methods: Data from 10,472 adults in the National Health and (...) Nutrition Examination Survey from 2013 to 2014 and 2015 to 2016 were analyzed for associations among prediabetes/diabetes risk factors, health care use, and hemoglobin A1C levels according to chi-square tests and multivariate logistic regression. Results: A total of 7.73% of US adults had seen a dentist but not a medical provider in the past 12 mo. The composition of this subpopulation was significantly different from that who saw a medical provider, in ways that might affect their diabetes risk. In addition, 31.27% of this subpopulation would be identified as being at high risk for prediabetes according to the CDC Prediabetes Screening Test, and 15.83% had hemoglobin A1C levels indicative of undiagnosed prediabetes or diabetes. Screening in a dental setting would have the highest odds of identifying someone unaware of his or her diabetes risk among those who were non-White, obese, or ≥45 y old. Conclusion: Extrapolation from this analysis indicates that screening for prediabetes at dental visits has the potential to alert an estimated 22.36 million adults of their risk for prediabetes or diabetes. Incorporating prediabetes or diabetes risk assessment into routine dental visits may enable 1) those with prediabetes to take action to decrease their risk of developing diabetes and 2) those with diabetes to engage in treatment to decrease their risk of diabetes-related complications. Knowledge Transfer Statement: Screening for prediabetes and diabetes during dental visits has the potential to raise patients’ awareness of diabetes risk and prevent prediabetes from progressing to diabetes. For some patients, the dental visit may be the only point of contact with the health care system, which heightens the importance of including diabetes risk assessment for patient well-being. AimWhite matter changes in individuals at ultra-high risk for psychosis (UHR) may be involved in the transition to psychosis. Sleep-wake disturbances commonly precede the first psychotic episode and predict development of psychosis. We examined associations between white matter microstructure and sleep-wake disturbances in UHR individuals compared to healthy controls (HC), as well as explored the confounding effect of medication, substance use, and level of psychopathology.MethodsSixty-four UHR individuals and 35 HC underwent clinical interviews and diffusion weighted imaging. Group differences on (...) global and callosal mean fractional anisotropy (FA) was tested using general linear modeling. Sleep-wake disturbances were evaluated using the subjective measures disturbed sleep index (DSI) and disturbed awakening index (AWI) from the Karolinska Sleep Questionnaire, supported by objective sleep measures from one-night actigraphy. The primary analyses comprised partial correlation analyses between global FA/callosal FA and sleep-wake measures. Secondary analyses investigated multivariate patterns of covariance between measures of sleep-wake disturbances and FA in 48 white matter regions of interest using partial least square correlations.ResultsUltra-high risk for psychosis individuals displayed lower global FA (F = 14.56, p < 0.001) and lower callosal FA (F = 11.34, p = 0.001) compared to HC. Subjective sleep-wake disturbances were significantly higher among the UHR individuals (DSI: F = 27.59, p < 0.001, AWI: F = 36.42, p < 0.001). Lower callosal FA was correlated with increased wake after sleep onset (r = −0.34, p = 0.011) and increased sleep fragmentation index (r = −0.31, p = 0.019) in UHR individuals. Multivariate analyses identified a pattern of covariance in regional FA which were associated with DSI and AWI in UHR individuals (p = 0.028), but not in HC. Substance use, sleep medication and antipsychotic medication did not significantly confound these associations. The association with objective sleep-wake measures was sustained when controlling for level of depressive and UHR symptoms, but symptom level confounded the covariation between FA and subjective sleep-wake measures in the multivariate analyses.ConclusionCompromised callosal microstructure in UHR individuals was related to objectively observed disruptions in sleep-wake functioning. Lower FA in ventrally located regions was associated with subjectively measured sleep-wake disturbances and was partly explained by psychopathology. These findings call for further investigation of sleep disturbances as a potential treatment target. (shrink)

If stem cell-based therapies are developed, we will likely confront a difficult problem of justice: for biological reasons alone, the new therapies might benefit only a limited range of patients. In fact, they might benefit primarily white Americans, thereby exacerbating long-standing differences in health and health care.

Smart grids are rolling out internationally, with the United States nearing completion of a significant USD4-plus-billion federal program funded under the American Recovery and Reinvestment Act. The emergence of smart grids is widespread across developed countries. Multiple approaches to analyzing the benefits of smart grids have emerged. The goals of this white paper are to review these approaches and analyze examples of each to highlight their differences, advantages, and disadvantages. This work was conducted under the auspices of a joint (...) U.S.-China research effort, the Climate Change Working Group Implementation Plan, Smart Grid. We present comparative benefits assessments of smart grid demonstrations in the U.S. and China along with a BA of a pilot project in Europe. In the U.S., we assess projects at two sites: the University of California, Irvine campus, which consists of two distinct demonstrations: Southern California Edison’s Irvine Smart Grid Demonstration Project and the UCI campus itself; and the Navy Yard area in Philadelphia, which has been repurposed as a mixed commercial-industrial, and possibly residential, development. In China, we cover several smart-grid aspects of the Sino-Singapore Tianjin Eco-city and the Shenzhen Bay Technology and Ecology City. In Europe, we look at a BA of a pilot smart grid project in the Malagrotta area west of Rome, Italy, contributed by the Joint Research Centre of the European Commission. The Irvine sub-project BAs use the U.S. Department of Energy Smart Grid Computational Tool, which is built on methods developed by the Electric Power Research Institute. The TEC sub-project BAs apply Smart Grid Multi-Criteria Analysis developed by the State Grid Corporation of China based on the analytic hierarchy process with fuzzy logic. The B-TEC and TNY sub-project BAs are evaluated using new approaches developed by those project teams. JRC has adopted an approach similar to EPRI’s but tailored to the Malagrotta distribution grid. Bioethics conjures images of dramatic healthcare challenges, yet everyday clinical ethics issues unfold regularly. Without sufficient ethical awareness and a relevant working skillset, clinicians can feel ill-equipped to respond to the ethical dimensions of everyday care. Bioethicists were interviewed to identify the essential skills associated with everyday clinical ethics and to identify educational case scenarios to illustrate everyday clinical ethics. Individual, semi-structured interviews were conducted with a convenience sample of bioethicists. Bioethicists were asked: (1) What are the essential skills required (...) for everyday clinical ethics? And (2) What are potential educational case scenarios to illustrate and teach everyday clinical ethics? Participant interviews were analyzed using qualitative content analysis. Twenty-five (25) bioethicists completed interviews (64% female; mean 14.76 years bioethics experience; 80% white). Five categories of general skills and three categories of ethics-specific skills essential for everyday clinical ethics were identified. General skills included: (1) Awareness of Core Values and Self-Reflective Capacity; (2) Perspective-Taking and Empathic Presence; (3) Communication and Relational Skills; (4) Cultural Humility and Respect; and (5) Organizational Understanding and Know-How. Ethics-specific skills included: (1) Ethical Awareness; (2) Ethical Knowledge and Literacy; and (3) Ethical Analysis and Interaction. Collectively, these skills comprise a Toolbox of Everyday Clinical Ethics Skills. Educational case scenarios were identified to promote everyday ethics. Bioethicists identified skills essential to everyday clinical ethics. Educational case scenarios were identified for the purpose of promoting proficiency in this domain. Future research could explore the impact of integrating educational case scenarios on clinicians’ ethical competencies. (shrink)

Meyer, Kent and Clifton (MKC) claim to have nullified the Bell-Kochen-Specker (Bell-KS) theorem. It is true that they invalidate KS's account of the theorem's physical implications. However, they do not invalidate Bell's point, that quantum mechanics is inconsistent with the classical assumption, that a measurement tells us about a property previously possessed by the system. This failure of classical ideas about measurement is, perhaps, the single most important implication of quantum mechanics. In a conventional colouring there are some remaining patches (...) of white. MKC fill in these patches, but only at the price of introducing patches where the colouring becomes ``pathologically'' discontinuous. The discontinuities mean that the colours in these patches are empirically unknowable. We prove a general theorem which shows that their extent is at least as great as the patches of white in a conventional approach. The theorem applies, not only to the MKC colourings, but also to any other such attempt to circumvent the Bell-KS theorem (Pitowsky's colourings, for example). We go on to discuss the implications. MKC do not nullify the Bell-KS theorem. They do, however, show that we did not, hitherto, properly understand the theorem. There is little doubt that Hume equivocates on the distinction between simple and complex impressions and ideas. Sometimes he identifies properties such as colors and shapes as simples. This is what he does, in fact, when he first introduces the distinction: Simple perceptions or impressions and ideas are such as admit of no distinction nor separation. The complex (...) are the contrary to these, and may be distinguished into parts. Tho' a particular colour, taste, and smell are qualities all united together in this apple, 'tis easy to perceive they are not the same, but are at least distinguishable from each other. Later, however, Hume speaks of simples not as qualities (or, of course, as substances) but as objects, i.e., things with properties: it is not red and round that are the simples but the red, round spot. He says, for example: Before I leave this subject I shall employ the same principles to explain that distinction of reason, which is so much talk'd of, and is so little understood, in the schools. Of this kind is the distinction betwixt figure and the body figur'd; motion and the body mov'd. The difficulty of explaining this distinction arises from the principle above explain'd, that all ideas, which are different, are separable. For it follows from thence, that if the figure be different from the body, their ideas must be separable as well as distinguishable; if they be not different, their ideas can neither be separable nor distinguishable (T 2425 ). 425 A genuine inconsistency with regard to the simple/complex distinction would have far reaching implications for Hume's system. Even the question concerning his understanding of necessity may be affected. It has been argued, for example, that what Hume believes separates the philosophical relations that are necessary from those that are not is that the former are internal relations. The suggestion is that the doctrine of internal relations, which is inextricably tied to a specific characterization of a simple/complex distinction, provides the essential contrast Hume needs for his ultimate denial that the causal relation is necessary. The consequences are compounded if we also take Robinson to be right in his attempt to reconcile Hume's 'conflicting' statements about causation in terms of the distinction between the natural and philosophical relations. While it is pointless to deny the ambiguity just set out (and, of course, one must remain constantly watchful to determine how Hume is employing the simple/complex distinction in a particular case), I believe a reconciliation of the two positions is still possible and that there is no genuine contradiction in Hume's system over this issue. The principle shift occurs between the distinction presented in logical terms and its presentation in psychological terms. In the latter instance what Hume seems to be worried about is Platonism: a metaphysics that countenances the existence of unexemplif ied qualities. This is most evident in the passage above where Hume, in effect, makes the following argument: if it were the case that (1) what is distinguishable in thought is separable in reality; and (2) figure and body are 426 distinguishable in thought; then it would follow that (3) figure and body are separable in reality. However, (3), i.e., Platonism, is false. But then so must be (2). Hume never deviates from this conclusion and its implications for his model of thought throughout the Treatise. It is this argument, for example, which became the basis for Hume's accusation against Locke's theory of abstraction and the foundation of his own view. So far Hume has it that neither thought nor experience present us with colors and shapes in isolation. This is his psychological point. Yet that need not be seen as conclusive with regard to what is to count as a logical simple. Building on his notion of a distinction of reason, Hume says: Thus when a globe of white marble is presented, we receive only the impression of a white colour dispos 'd in a certain form, nor are we able to separate and distinguish the colour from the form. Perhaps the greatest irony about the contemporary religion‐science dialogue is the fact that, despite their own strongly articulated denials, many thinkers implicitly accept the “warfare” thesis of A. D. White—that is, they agree with White that traditional theology has proved unable to engage science in fruitful conversation. On 6 September 1836, George White wrote from Hatton Garden to T. B. Hall in Liverpool:I see by an advertisement that [there is] a proposition to form a Society to be called the Botanical Society of London—Its objects are the advancement of Botanical Science in general but more especially systematic and descriptive Botany—the formation of a Library, Museum & Herbarium—A meeting will be held at the Crown & Anchor, Strand, tomorrow evening & it is my intention to attend it—It (...) has been proposed thatLadiesshould be admitted!!!If the writer of those words lived up to his declared intention and did attend that or any other of the long string of inaugural meetings the Society held during the last quarter of that year, he would have been startled, perhaps even appalled to find how seriously that last-mentioned proposal had been taken. For on 3 November he would have found in the room at the Crown and Anchor Tavern (according to one report) ‘a crowded assembly of both ladies and gentlemen’. He would also have heard the founder of the Society, the nineteen-year-old Daniel Cooper, deliver a paper on the effects of light on plants, which (according to the same report) ‘excited great interest, more particularly with the ladies’. A fortnight later the meeting was again ‘numerously attended’ and again it attracted a number of the supposedly unlearned sex, some of whom by then were ‘members of the society’ unambiguously. One aim of this essay is to understand why white evangelical Christians, more than any other religious adherents in the United States, are deeply invested in denying the emergency of anthropogenic climate change and in obstructing action to address anthropogenic climate change. Michael S. Hogue, in his recent book, American Immanence, blames a religious imaginary he names the “redeemer symbolic.” This symbolic complex inspires the devotion of the politically powerful white evangelical Christian and nationalist movement in the United (...) States at the present time. A second aim of the essay is to analyze the redeemer symbolic. Through a reading of Maurice Sendak’s much-loved illustrated children’s book, Where the Wild Things Are, the essay suggests that U.S. white evangelical devotees of the redeemer symbolic share a kind of inability to come to terms with a vital and ineliminable wildness in persons and cultures; further, that this inability correlates with a political-theological failure, even refusal, to grasp the emergency of anthropogenic climate change. The essay first explicates the redeemer symbolic, with a particular focus on its implication in the legitimation of climate skepticism. Then, with the aid of key concepts from the psychoanalytic theory of D.W. Winnicott, it interprets the story of Max, the protagonist in Where the Wild Things Are, as a fable of healthy development of what Winnicott calls “transitional space” and a related “capacity to be alone.” Unsuccessful development of those resources, it is suggested, contributes to an account of why adherents of the redeemer symbolic typically refuse wildness and thus may be prone to climate negligence. More importantly, though, recognizing the cultural-psychological importance of “wild things” may help fire imaginative ways around the obstructiveness of the redeemer symbolic, to more effectively address climate change in particular, and human well-being in nature in general. Upon entering the White House in 1933, President Franklin D. Roosevelt faced an ailing economy in the throes of the Great Depression and rushed to transform the country through recovery programs and legislative reform. By 1934, he began to send professional photographers to the state of West Virginia to document living conditions and the effects of his New Deal programs. With over one hundred and fifty images by ten FSA photographers, including Walker Evans, Marion Post Wolcott, Arthur Rothstein, and Ben Shahn, this collection is a remarkable proclamation of hardship, hope, endurance, and, above all, community. This slim volume is another addition to the ever growing family of anthologies—in this case Oxford Readings in Philosophy—recognizable as such not only because of its eye-catching external appearance but also, and mainly, on account of its close similarity in technical features to the previously born siblings. The composite body of the various contributions bears the unmistakable mark of ‘analysis’ as an identifying label. This observation, needless to say, is not intended to be condemnatory—for there is a great deal to (...) be said in favour of dissecting propensities—it is merely an indication of what one should expect to find in this compilation. The nonet playing ‘Action’ consists of the following performers: J L Austin, A C Danto, H A Prichard, A I Melden, D Davidson, J Feinberg, P J Fitzgerald, G E M Anscombe, J O Urmson. As can be seen, the group represents a pretty homogeneous bunch—with the possible exception of the veteran ethical intuitionist Prichard, whose participation ceases to be a mystery only after Melden’s essay has been digested. I must confess that I find the arrangement of the articles rather puzzling. There seems to be no clear criterion involved. It cannot be in order of importance ; neither can it be taken to signify logical affinity ; it certainly is not chronological; and—in the presence of an eminent editor—one hesitates to see it as an ad hoc, haphazard motley. It might be maintained that A White’s excellent introduction—a rare combination of lucidity, economy and relevant information—with its appropriate headings throws some light on the collocation of players in the ensemble. If it was intended to do this then I fail to see the logic behind the plan. This, however, is a minor matter when compared with the meritorious execution of a difficult task which the introduction represents. It gives us a comprehensive bird’s-eye view of the territory disputed and pin-points neatly the various claimants to the, often more than obscure, land-titles. Considering the nature of the landscape and the diversity of figures swarming over and about it Professor White has done a splendid job and deserves unqualified praise. (shrink)

In the context of the motor skill literature on observational learning and hierarchical skill structuring, Byrne & Russon's findings call into question their standpoint that great apes imitate the behaviour of role models at the programme level. The authors impose a hierarchical model on their observations without properly considering alternative explanations. One such possibility, which stems from a constraints perspective that they dismiss, is put forward.

Informed consent is one of the foundational ethical and legal requirements of research with human subjects. The Nuremberg Code, the Helsinki Declaration, the Belmont Report, the Common Rule and many other laws and codes require that research subjects make a voluntary, informed choice to participate in research.12345 Informed consent is based on the moral principle of respect for autonomy, which holds that rational individuals have a right to make decisions and take actions that reflect their values and preferences. 6 Whereas (...) most guidelines and codes also require that informed consent be properly documented, informed consent is much more than signing a piece of paper: It is a continuous process of communication between the investigator and the research subject. 7 Because the body of knowledge impacting a study frequently changes, subjects should receive information from investigators after they have enrolled in a study, such as significant new findings that may affect their decision to participate in research or clinically useful tests results. 8910 In large studies, some investigators use newsletters to update subjects on the progress of research and other developments.11Most of the ongoing communications between investigators and subjects involve little more than information sharing, without revisiting the decision to participate in research or signing any additional documents. Sometimes, however, it may be necessary for subjects to reaffirm their decision to participate, to re-consent, or to sign or re-sign a document. 8 Re-consent can be defined as an action in which a subject makes the decision to participate in research once again. Re-consent is different from reaffirming a commitment to participate in a study, because in re-consent one actually reconsiders the information necessary to make decision, whereas in reaffirmation one simply expresses ….
